Job summary

Evolve Therapy Services in New Haven, CT is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. This role supports residents under the supervision of a licensed therapist and requires CT OTA licensure.

You will conduct screenings, implement treatment plans, document outcomes, coordinate with the care team, and participate in CMS-compliant documentation and billing. Full-time, part-time, or PRN schedules may be available.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy is required.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention per scope of practice.
• Perform treatment plans and follow goals under supervision of a therapist.
• Communicate findings to residents, families, and the interdisciplinary team.
• Schedule individual resident therapy with team cooperation.
• Conduct therapy per plan and ensure proper documentation and billing.
• Perform other duties to effectively treat patients.
• Comply with CMS documentation standards daily.

Job Position:

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Type:

Full Time, Part Time or PRN positions available

Job Purpose:

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions:

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications:

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
