Qualifications

• Comprehensive assessment skills focused on ADLs and IADLs.
• Ability to develop individualized care plans.
• Familiarity with Section GG data collection and documentation.

Responsibilities

• Perform assessments of residents' functional abilities.
• Develop tailored intervention plans for residents.
• Ensure accurate Section GG data collection.
• Administer OT interventions and adaptive equipment provision.
• Educate residents and family members on mobility techniques.
• Maintain detailed documentation in EMRs.
• Collaborate with other rehab staff for holistic care.
