Job summary

Relient Health is seeking a full-time Occupational Therapist (OT) for a Skilled Nursing Facility (SNF) in Elk Grove, CA. This SNF setting offers great pay, full benefits, and a warm, supportive team.

New grads are welcome to apply. You will evaluate patients, create individualized treatment plans, deliver interventions to improve function, document progress, and collaborate with an interdisciplinary team, supervising COTAs as needed.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current CA license (or eligibility).
• Passionate about improving patient quality of life.
• Strong communication and teamwork skills.

Responsibilities

• Evaluate patients and create individualized treatment plans.
• Deliver therapeutic interventions to improve function and independence.
• Document progress and adjust plans as needed.
• Educate patients, families, and caregivers on therapy goals.
• Collaborate with an interdisciplinary care team.
• Provide supervision and guidance to COTAs.
