Job summary

Title

Occupational Therapist (OT)

Job Summary

A licensed Occupational Therapist professional who provides coordinated care to patients, evaluates patients, and restores or maintains patient well‐being according to the plan of care.

Essential Functions
  • Evaluate and provide a treatment plan for a patient with physical, psycho‑social, and/or cognitive impairments.
  • Provide intervention for deficits and elicit responses.
  • Collaborate with interdisciplinary team.
  • Perform initial and ongoing clinical assessments to identify patient needs, determine level of function at appropriate time points, and assess appropriateness for home health.
  • Develop and revise a plan of care in consultation with physician and other care team members.
  • Provide therapeutic treatments, evaluate need for and use of adaptive equipment, perform environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable.
  • Evaluate outcomes of care with appropriate discharge planning.
  • Apply infection control and standard precautions during patient care activities to prevent contamination and transmission of disease.
  • Supervise COTA as needed.
  • Complete appropriate documentation in a timely manner.
  • Meet mandatory continuing education requirements of agency/license board.
  • Use effective interpersonal relations and communication skills to advise and consult with family and other agency personnel.
  • Demonstrate commitment, professional growth and competency.
  • Promote agency philosophy and administrative policies to ensure quality of care.
Job Qualifications
  • Education: Bachelor of Science in Occupational Therapy.
  • Licensure: Current Texas State License as an Occupational Therapist.
  • Experience: 1 year minimum in Home Healthcare, Acute care, or Rehabilitation setting.
  • Therapy skills as defined by generally accepted standards of practice.
  • Good communication and interpersonal skills.
  • Proof of current CPR and Hepatitis profile.
  • Knowledge of durable medical equipment.
  • Spanish language skills are a plus.
