BAYADA @ Bayhealth is seeking a full-time Occupational Therapist (OT) for home health visits and assessments in Sussex County, serving adult and geriatric patients in their homes.

The OT will be part of a multidisciplinary team providing rehabilitative and skilled nursing care, enabling clients to remain at home.

Occupational Therapist, OT Responsibilities:
  1. Make home visits to clients in designated areas.
  2. Perform evaluations of patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL/IADL performance.
  3. Assess cognitive, psychological, and motor performance; develop individualized intervention plans.
  4. Evaluate home environments for hazards or barriers.
  5. Identify equipment needs and environmental adaptations.
  6. Continuously assess and revise the care plan; collaborate with nursing in the multidisciplinary care plan.
  7. Educate patients and families on rehabilitative care and activities.
  8. Document observations, interventions, and evaluations accurately using a touch pad tablet.
Occupational Therapist, OT Qualifications:
  • Current DE Occupational Therapist license.
  • Graduation from an accredited occupational therapy program.
  • Ability to work independently and manage time effectively.
  • Strong interpersonal, organizational, and problem-solving skills.
  • Solid computer skills; EMR experience preferred.
