Position Summary

The Home Health Occupational Therapist - PPV PRN models the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation, and promotes the Compassus philosophy based on the 6 Pillars of Success. The therapist upholds the Code of Ethical Conduct and fosters positive relationships within the organization and with external stakeholders. Responsibilities include providing quality rehabilitative occupational therapy to clients, initiating planned activities to enhance physical and psychosocial functioning.


Position Specific Responsibilities

  1. Develops occupational therapy care plans for patients.
  2. Conducts assessments and evaluations during visits; communicates with physicians and completes OASIS assessments as needed.
  3. Implements care plans and provides education to team members, patients, and families.
  4. Evaluates care outcomes and supervises paraprofessionals.
  5. Plans for patient discharge when appropriate.
  6. Participates in quality improvement programs.
  7. Supports company growth through resource management and community education.
  8. Attends interdisciplinary team meetings for coordinated patient care.
  9. Follows agency policies, procedures, and regulatory rules.
  10. Accepts direction from Clinical Leadership and meets productivity standards.
  11. Communicates with physicians and documents patient information timely in EMR.
  12. Completes all documentation accurately and promptly.
  13. Provides skilled restorative treatments, including joint/muscle function, pain management, sensory integration, orthotic fabrication, and safety evaluations.
  14. Ensures patient rights are respected and reports grievances or abuse allegations.
  15. Adheres to infection control, safety protocols, and emergency procedures.
  16. Educates staff on psychosocial aspects as needed.
  17. Participates in policy development and maintains confidentiality.
  18. Maintains proper documentation for billing and ensures customer satisfaction through effective communication.
  19. Supervises Occupational Therapy Assistants and Home Health Aides.
  20. Maintains regular attendance and performs other duties as assigned.


Education and Experience

  • Bachelor of Science in Occupational Therapy required.
  • At least one year of relevant experience preferred.


Skills

  • Strong mathematical, language, and communication skills.
  • Ability to understand and articulate hospice philosophy and work effectively in a team.


Certifications, Licenses, and Registrations

  • Active, unencumbered Occupational Therapist license in the state of employment required.


Physical Demands and Work Environment

Requires standing, walking, manual dexterity, lifting up to 50 pounds, and visual acuity. Exposure to bodily fluids and infectious diseases is expected. Proper use of medical equipment is essential. Accommodations can be made for disabilities. This overview may vary by role and department.
