As an Occupational Therapist, you will evaluate clients' physical functioning in the home, plan, coordinate, and provide occupational therapy services as prescribed by a physician. You will collaborate with other professionals to establish rehabilitation and maintenance goals.

Responsibilities include:
- Developing treatment plans that include modalities, visit frequency, expected outcomes, and goals.
- Assessing patients and creating therapeutic care plans for activities of daily living.
- Evaluating home environments and recommending modifications for safety and energy conservation.
- Assessing patients' ability to perform daily activities and recommending strategies to enhance functionality.
- Recommending medical equipment and adaptive tools for ADL.
- Considering psychological responses to illness and physical changes.
- Setting realistic short-term and long-term goals in treatment plans.
- Documenting evaluations, treatment plans, and progress.
- Implementing treatment plans under physician guidance and collaborating with team members.
- Teaching and monitoring exercises to improve strength, range of motion, and coordination.
- Instructing on the use of adaptive equipment for independence.
- Providing education for maintenance programs to patients and families.
- Counseling patients and families to aid adjustment to functional limitations.
- Monitoring patient reactions and reporting changes to physicians.
- Developing alternative plans for ineligible referrals.
- Maintaining communication with care providers to support rehabilitation.
- Adjusting treatment plans as needed and documenting modifications.
Qualifications:
- Current license and registration to practice as an Occupational Therapist in New York.
- Membership with the American Occupational Therapy Association or graduation from an approved Occupational Therapy program.
- At least 1 year of experience in hospital, long-term care, rehab, or home health settings.
- Experience with pediatric populations is required; experience with diverse age groups is a plus.
- Knowledge of Medicare/Medicaid and regulatory standards.
- Ability to perform home visits in potentially unsafe environments, comfortable with travel, and able to handle physical demands, including lifting up to 50 pounds unassisted and up to 325 pounds with assistive devices.
