Job summary

Concept Rehab in Warren, MI is seeking a dedicated Occupational Therapist to join our post-acute rehabilitation team full time. You will evaluate residents, develop individualized treatment plans, and help patients regain daily living skills to return to the highest possible level of independence.

Qualifications

• Active Michigan Occupational Therapist license.
• Degree from an accredited Occupational Therapy program.
• Strong clinical judgment and communication skills.
• Experience in skilled nursing, rehabilitation, or geriatrics preferred.

Responsibilities

• Complete occupational therapy evaluations and establish individualized treatment programs.
• Provide skilled interventions addressing daily living activities, upper extremity function, coordination, and safety.
• Assist residents with improving dressing, bathing, grooming, and other functional skills.
• Recommend adaptive equipment and compensatory techniques when appropriate.
• Monitor progress and modify treatment plans based on resident needs.
• Complete accurate and timely clinical documentation.
• Work collaboratively with nursing, therapy, and other healthcare professionals.
