Job summary

Wilmington Nursing and Rehabilitation Center is seeking an Occupational Therapist (OT) to join our compassionate team. The ideal candidate will have an active state license, be dependable, self-motivated, a team player, and flexible to meet residents' needs.

This Full Time role may also require PRN shifts to support staff coverage. OTs will assess residents, develop and administer treatment plans, document progress, and collaborate with all disciplines to provide patient-centered care in a

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred.

Responsibilities

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
