Renewal Rehab is seeking a licensed Occupational Therapist for a full-time position in Chicago Ridge, IL. You will evaluate and treat patients in a skilled nursing/long-term care setting, develop care plans, and collaborate with a multidisciplinary team to achieve optimal outcomes.

Qualifications

• Must be a licensed Occupational Therapist to apply.
• NBCOT certification may be required by state.
• Current license as an Occupational Therapist in the state of practice.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all physician orders are obtained and up to date.
