Job summary

Tender Touch Rehab Services LLC is hiring a Licensed Occupational Therapist for a full-time position in a Skilled Nursing Facility and Long-Term Care setting. You will evaluate patients, develop and implement care plans, and collaborate with a multidisciplinary team while maintaining regulatory documentation.

Qualifications

• Degree in Occupational Therapy from an AOTA-accredited program.
• Current license as an Occupational Therapist in the state of practice.
• NBCOT certification may be required by the state.
• Strong communication and organizational skills.

Responsibilities

• Evaluate and treat patients with functional, cognitive, and ADL impairments.
• Create and implement personalized care plans.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
