Job summary

Evolve Therapy Services, LLC in Selma, AL is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility. You will implement and monitor individualized therapy plans under supervision, contribute to interdisciplinary care, and support residents’ functional goals.

The role requires an Associate's degree in Occupational Therapy, active OTA licensure in applicable states, and ongoing continuing education.

Qualifications

• Associate's degree in Occupational Therapy required.
• Must hold active OTA licensure in applicable states.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine interventions.
• Perform treatment plans and follow goals established by supervising therapist.
• Interpret and communicate evaluation findings to residents, families, and team.
• Establish individual resident therapy schedules with team members.
• Provide therapy according to plan and document/bill services.
• Perform other duties to effectively treat patients as assigned.
• Comply with CMS documentation standards daily.

Education

Associate's degree in Occupational Therapy

Active OTA licensure in state(s) of practice

Ongoing continuing education and professional development
