Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant in Mobile, AL for an in-house Skilled Nursing Facility. This PRN role supports plan-based OT services to residents and collaborates with a multidisciplinary team.

Responsibilities include screenings, treatment delivery, documentation, and adherence to CMS standards. Active OTA licensure and ongoing continuing education are required to maintain licensure and provide quality patient care.

Qualifications

• Must have active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine intervention or treatment.
• Perform treatment plan to residents; follow short- and long-term goals per plan of care.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents and teams.
• Establish individual resident therapy schedules with team members.
• Conduct therapy according to plan and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily with point of care documentation.

Education

Job Position

Certified Occupational Therapy Assistant for an In-house Skilled Nursing Facility

Job Purpose

To plan, organize, develop, and direct Occupational Therapy Services in facility in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be directed by Administrator, to ensure that highest degree of quality resident care can be maintained at all times.

Job Functions

• Conduct regular screening of residents to figure out the best intervention or treatment for each individual patient based off scope of practice
• Perform treatment plan to residents; follow short- and long-term goals and methods to achieve goals based off of the established plan of care by supervising therapist
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members
• Establish individual resident therapy schedules in cooperation with other team members
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services
• Perform all other duties as assigned and as required to effectively treat patients
• Comply with all CMS documentation standards of practice daily with point of care documentations as indicated

Education & Qualifications

• Associate's degree in Occupational Therapy
• MUST have active OTA licensure in state(s) of practice
• Ongoing continuing education and professional development to maintain current licensure and certification
