Job summary

Rivetus Rehabilitation is seeking an Occupational Therapy Assistant to join the Take Your Therapist Home program. You will work in Home Health with SNF collaboration, maintaining a dedicated local caseload and a traditional workweek.

You will use professional judgment, adapt to patient and family needs, and lift/move patients with appropriate devices within a hybrid Home Health and SNF setting. Michigan licensure is required.

Qualifications

• Requires successful completion of an accredited OTA program.
• Licensure as required by the State of Michigan.

Responsibilities

• Provide Home Health therapy in conjunction with SNF‑based services.
• Manage a localized caseload within the surrounding area of the SNF.
• Maintain professional judgment and care standards while working in a hybrid home health/SNF setting.
