Job summary

The role combines SNF and home health assignments with a hybrid work arrangement, centered around a traditional Monday–Friday schedule and a localized service area.

Eligible candidates have an accredited OTA program and Michigan licensure, with opportunities to grow

Qualifications

• Graduation from an accredited OTA program.
• Licensure as required by the State of Michigan.

Responsibilities

• Work directly with patients in the SNF and home health.
• Provide continuity of care across SNF, home health, and outpatient settings.
• Serve as a clinician in the home health setting with SNF caseload support.
• Maintain a localized coverage area surrounding the SNF.
• Follow a traditional Monday–Friday work week.
