Job summary

Lakeshore Bone & Joint Institute in northwest Indiana seeks a licensed Physical Therapist for our outpatient orthopedic team across Chesterton, Crown Point, Michigan City, Portage and Schererville.

You will evaluate, plan, and administer PT treatments, supervise support staff, and collaborate with physicians to restore function and mobility. This role supports a four-day work week with dedicated time for education and mentorship.

Qualifications

• Current, valid state Physical Therapist license.
• DPT preferred.
• BLS certification.
• Minimum of one year of outpatient orthopedic PT experience; strong new graduates considered.

Responsibilities

• Conduct initial evaluations to assess condition and history.
• Formulate evidence-based treatment plans with measurable goals.
• Administer manual therapy, therapeutic exercises, and modalities.
• Track progress and adjust plans to optimize recovery.
• Educate patients and families on home exercises and prevention.
• Maintain EMRs; document evaluations, daily notes, progress, and discharge summaries.
• Coordinate care with physicians, surgeons, and other professionals.
• Supervise PTAs and rehabilitation aides as permitted.
• Review history, establish personalized goals, and guide prescribed exercises.
