Summary:

The Orthopedic Patient Navigator provides care coordination to Orthopedic patients with the goal of overcoming potential or realized barriers to care, providing education and resources to facilitate timely delivery of services, and improving patient outcomes. The Orthopedic Patient Navigator collaborates with the multidisciplinary team as an advocate for inpatients and outpatients across the continuum of care, building relationships, and supporting patients. The Orthopedic Patient Navigator serves as a liaison throughout the facility and in the community regarding services provided for the Orthopedic patient population. The Orthopedic Patient Navigator works to develop and grow strong Orthopedic Programs under the Orthopedic Service Line.

Responsibilities:

  • Identifies potential and realized barriers to care and facilitates communication and referrals as appropriate to mitigate barriers.
  • Responsible for following the patient and being a resource throughout the continuum of care
  • Provides and/or reinforces education to patients, families, and caregivers about diagnosis, surgery, and post discharge care.
  • Responsible for providing pre op education classes and discharge education classes.
  • Develops and updates educational resources as needed, utilizing best practice and research findings (i.e. pre op and discharge class, and guidebooks).
  • Ensures that all education materials are standardized and consistent with standard order sets.
  • Ensures that clinical pathways are updated annually to maintain high quality of care standards.
  • Participates in staff education on an individual and or group basis as needed and provides support to the Orthopedic team for continual learning.
  • Works in collaboration with marketing and outreach to conduct community education seminars in targeted outreach areas to increase awareness and grow market share. Assists with physician-based community education seminars as needed.
  • Participates in coordination of the plan of care with the multidisciplinary team promoting timely follow up on treatment and supportive care recommendations. Acts as a liaison and advocate between patients, families, caregivers, physician offices, and providers, to ensure timeliness of diagnostic services, completion of preoperative work ups, and optimization of patient outcomes.
  • Collaborates with physicians, nursing, physical therapy, operating room staff, marketing department, food service, pharmacy, outreach programs and all members of the healthcare team through team conferences and one-on-one meetings to ensure a quality care –delivery process for patients across the continuum
  • Builds relationships with physicians and other healthcare providers to establish and maintain referrals to the orthopedic programs.
  • Tracks metrics and patient outcomes to document and evaluate outcomes of the programs.
  • Provides reports of established metrics and others as requested.
  • Accountable for the key performance measures of the Orthopedic Service Line. Works with the team to identify and track key performance measures including but not limited to patient satisfaction, physician satisfaction, LOS, cost per

case, discharge disposition, and orthopedic specific outcome tools as necessary. Works with the data and distributes reports to key stakeholders and participates in process improvement initiatives as required.

  • Maintains a current knowledge of changes in Orthopedic patient care management standards and takes appropriate measures to incorporate into performance systems.
  • Promotes evidence-based practice in self and others by reading, interpreting and evaluating research for utilization in practice.

Credentials:

  • Valid New Jersey nursing license (RN)
  • Orthopedic Nursing Certification preferred

Education or Equivalent Experience:

  • Bachelor Degree in Nursing is required (Master's Degree in nursing preferred)

Salary Range:

$92,022.74 - $133,674.74/Annually based on 40 hours per week
