Banner Health in Loveland, Colorado is seeking a Physical Therapist to join the Outpatient Rehab Therapy Clinic. The role focuses on adult patients, including orthopedic and neurological populations. Outpatient Ortho experience is preferred but not required.

Qualifications

• Must have graduated from an APTA accredited school of Physical Therapy, or equivalent.
• Must possess a current Physical Therapy license within the State of practice. BLS certification required.
• Ability to become proficient with computer software applications commonly used in an automated patient care environment.
• Arizona Fingerprint Clearance Card at time of hire and maintain the card for duration of employment.

Responsibilities

• Evaluates patient’s functional abilities; establishes plan of care and treatment goals.
• Implements and adjusts individualized treatment plans; educates patient and family.
• Documents according to guidelines; ensures timely and accurate records.
• Communicates results to physicians, nurses, patient and interdisciplinary team.
• Supervises personnel to ensure safe, cost-effective patient care.
• Participates in department meetings and process improvements.
• Interacts with multiple staff and external contacts to coordinate care.
