Qualifications

• Oral and Maxillofacial Surgery Residency Certificate is required.
• DDS or DMD from an accredited dental school is required.
• Current Oklahoma dental license or ability to obtain one prior to employment is required.
• Current or ability to obtain required credentials and certifications, including CPR, DEA, and NPI, is required.
