Job summary

Kulanu Academy in Cedarhurst, NY is seeking a Job Coach to support participants with learning and job skills through on-site coaching, tailored plans, and outcomes.

You will document SAP plans, act as liaison between the person supported, family, and employer, and participate in staff meetings and trainings. A high school diploma is required; college degree preferred and a NYS driver's license is needed.

Qualifications

• High School Diploma or equivalent.
• College degree preferred.
• Valid NYS Driver’s License.
• Experience in working with individuals with special needs.
• Knowledge of OPWDD regulations is a plus.

Responsibilities

• Provide on-site job coaching for designated caseload.
• Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies DDPS and other required information.
• Act as a liaison between the person supported, family and employer.
• Attend all staff meetings, Life Plans, conferences, and annual training requirements.
• Ability to demonstrate patience, creative thinking, and exhibit great teamwork.
• Provide overall assistance as assigned.

Description

The Job Coach specializes in assisting participants with learning and gaining job skills. The Job Coach provides one-on-one job training tailored to the needs of the participant.

The Job Coach is responsible for but not limited to the following:

• Provide on-site job coaching for designated caseload
• Complete and ensure accurate and timely documentation of SAP plans, case notes, monthlies DDPS and other required information.
• Act as a liaison between the person supported, family and employer.
• Attend all staff meetings, Life Plans, conferences, and annual training requirements.
• Ability to demonstrate patience, creative thinking, and exhibit great teamwork.
• Provide overall assistance as assigned.
