We are seeking an Optometrist to join us in pursuit of high-quality patient care to provide Optometry
services including but not limited to diagnosis, treatment, coordination of care, preventive care and health
maintenance.


Principal Accountabilities

  • Diagnose and non-invasively treat a wide variety of conditions and diseases of the human eye
    and visual systems.
  • Review patient ocular and medical histories and reports and perform physical examination.
  • Examine eyes using observation, instrument and pharmaceutical agents, to determine visual
    acuity and perception, focus and coordination, and to diagnose acute and chronic eye diseases,
    minor ocular trauma and other ocular conditions requiring urgent management.
  • Maintain complete and accurate medical records.
  • Analyze test results and develop ongoing treatment and disease management plans.
  • May prescribe therapeutic medications within the Optometrist’s legal scope of practice to treat
    specific eye conditions.
  • Provide care for patients undergoing eye surgeries, such as cataract and laser vision correction,
    including pre- and post- operative care.
  • Diagnose problems or impairments and prescribe corrective lenses.
  • Prescribe therapeutic measures to correct or conserve vision such as low vision rehabilitation.
  • Adhere to best practices and follow established quality measures for the prevention of eye
    disease, including but not limited to, performing HEDIS diabetic eye exams.
  • Recommend suitable management for the patient, including but not limited to, preventive health
    recommendations and habits.
  • Educate and counsel patients on contact lens care, visual hygiene, lighting arrangements and
    safety factors. May counsel patients regarding surgical and non-surgical options to meet their
    visual needs.
  • Provide relationship-centered care, taking into account the patient’s psychosocial and physical
    needs.
  • Consult with and refer patients to ophthalmologists or other health care practitioners if additional
    medical or surgical treatment is determined to be necessary.
  • Communicate effectively with the clinical team to ensure total delivery of quality care.
  • Participate in medical staff educational programs and meetings.

Education, Training, Licenses, Certifications

  • Doctor of Optometry (O.D) from an accredited Optometry School.
  • Must hold current TPA (Therapeutic Pharmaceutical Agents) Certification and DPA.
    (Administration of Topical Diagnostic Pharmaceuticals) Certification.
  • Seeking both experienced – 2 years or above and newly trained providers.
  • Must pass National Board of Examiners in Optometry (NBEO) exam.

Relevant Work Experience, Knowledge, Skills, and Abilities

  • Must possess current, unrestricted New York State Optometrist license.
  • Must possess active Medicare/Medicaid Enrollment or be eligible to enroll.
  • Basic Life Support (BLS) (Required) or Advanced Cardiac Life Support (ACLS) (Preferred).
  • Excellent understanding of HEDIS quality measures related to Ophthalmology.
  • Working knowledge of pharmacology and evidence-based treatments for eye conditions and
    diseases.
  • Must possess strong communication skills both oral and written.
  • Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.
  • Demonstrate competency in diagnosis and treatment to assure high standards of patient care and
    safety are maintained.
  • Interest in Population Health Management.
