Peninsula Agency on Aging in Newport News is seeking an Options Counselor for the Veteran's Directed Care (VDC) Program to provide case management and service coordination for eligible clients.

The role focuses on advising veterans and caregivers about available services, conducting assessments, offering options counseling, and creating comprehensive care plans to support aging in place and independence.

Qualificações

• Bachelor's Degree from accredited college or university in social work, human services, or related field.
• Experience with working with seniors and/or disabled adults.

Peninsula Agency on Aging (PAA) is currently seeking an individual to serve as the Options Counselor for the Veteran's Directed Care (VDC) Program to provide case management and service coordination for eligible clients.

Key Responsibilities

As the Options Counselor for the VDC Program, you will be instrumental in advising veterans and/or their caregivers of the services and resources available to support aging in place, and utilize interview skills to screen and complete assessments, provide options counseling, and develop comprehensive plans of care/service plans for veterans.

Essential Skills and Experience

We are looking for applicants with any combination of education and experience equivalent to a Bachelor's Degree from accredited college or university in social work, human services, or related field. Applicant needs to be experienced with working with seniors and/or disabled adults, case management, excellent interviewing, oral, and written communication skills, and effective use of computers, software, and mobile technology. Applicants must also have the ability to model our values, and be vaccinated for COVID-19.
