POSITION: Responsible for facilitating access to services, managing and communicating service-related data to third parties, coordinating team communication with outside entities, and ensuring retention in treatment. This position supports clinical services by taking a lead role in actively encouraging a team approach to treatment.
QUALIFICATIONS: A Master's degree in counseling, social work, or a related field. Or a Bachelor's degree with training and experience in the mental health/drug & alcohol fields. Must have good oral, written and computer skills. Ability to work independently. Act 33, Act 34, and FBI Fingerprint Clearances required.
DUTIES/RESPONSIBILITIES
- 1. Process intakes from the initial call to assignment for outpatient services including opening charts and completing the triage. Clinical review with Clinical or D&A Director for case assignments if necessary.
- 2. Completion of required trainings: assessment, Drug & Alcohol, and procedural.
- 3. Completion of Biopsychosocial assessments and MHAs for Open Access, including assignment of diagnosis, level of service, and other recommendations for treatment.
- 4. Track the OA process, assuring each client completes the process. (included but not limited to; registration, WITS, assessments, assignment and connection to services).
- 5. Understanding of crisis situations requiring further assessment for hospitalization. Ability to learn and apply suicide and homicide risk assessment tools and procedures and make recommendations accordingly.
- 6. Knowledge of liability determinations, ability to complete liabilities and registrations, and must remain current on any changes to agency liability process.
- 7. May attend treatment team meetings as needed to assist in facilitating case assignments for complex cases.
- 8. Covers for OPMH and/or D&A therapists when they are unavailable.
- 9. Manages a caseload of clients and provides individual and/or group therapy in the OPMH and/or D&A programs.
- 10. Responds to requests for information on programs and services both internal and external on a daily basis.
- 11. Entering of D&A information into the state's WITS system
- 12. Work at satellite offices is required.
- Maintains and upholds Community Counseling Center's policies, respects confidentiality, and exhibits a professional demeanor at all times while at work.
- Adheres to agency policies and procedures/job duties or will be subject to disciplinary action.
- Performs other duties as assigned.

- Participates in CCC CQI plan to ensure quality of service to CCC consumers.
