Responsibilities:
- Demonstrate accountability for professional development and actively participate in practice-based governance, goal setting, and quality improvement initiatives. Serve as an engaged team member, responding to care team needs, and supporting change processes. Provide oversight and supervision within the practice environment, including leadership with new staff and coordination of clinical and administrative activities.
- Formulate patient care goals and plans that involve patients as partners, considering their holistic needs. Triage patient situations, coordinate care with physicians, assist with assessments, evaluations, and education, and manage outpatient follow-up. Act as the primary nursing resource for your caseload and facilitate necessary services outside the healthcare facility.
- Develop and maintain productive relationships with colleagues and external partners, demonstrating accountability, enthusiasm, and cultural competence. Promote safe work hours, effective time management, and healthy lifestyles. Communicate safety concerns and assist in care planning and operational improvements.
- Participate in quality improvement efforts, identify opportunities for enhancement, and support patient satisfaction initiatives. Utilize evidence-based practices and assist in regulatory and quality audits. Advocate for patients and serve as a liaison within the practice.
- Establish positive relationships with staff, providers, and patients. Handle multiple priorities effectively, exercise clinical judgment, and demonstrate critical thinking and problem-solving skills. Maintain physical stamina for frequent movement and patient handling.
- Apply critical thinking to identify and address clinical, social, safety, and psychological issues. Communicate effectively about patient conditions, document thoroughly, and incorporate professional standards to improve safety, quality, and satisfaction.
- Utilize adult learning principles to educate patients, families, students, and new staff. Provide comprehensive teaching, serve as a preceptor, and support staff development. Foster an environment of open dialogue and continuous improvement.
