UPMC Community Medicine Incorporated is hiring a full-time Outpatient Nurse Coordinator to support Burke and Bradley Orthopedics located at
200 Delafield Road, Suite 4010
Pittsburgh, PA 15215. This role will work Monday through Friday daylight hours (varies from 7:30am to 4pm, 8am to 4:30pm and can run over if clinical hours run over). This position will require travel to UPMC East (Monroeville) on Thursdays.
Responsibilities:
- Demonstrates accountability for professional development that improves the quality of professional practice and patient care. Participates in practice-based shared governance, goal setting, and supports change to improve care quality. Acts as a full partner on the care team, responding to team needs and participating in quality improvement efforts. Provides oversight and supervision, demonstrating leadership with new staff and managing patient processes.
- Formulates goals and plans of care involving the patient as a partner, considering individual needs holistically. Triages patient situations, coordinates care with physicians, and assists in assessment, evaluation, and education. Responsible for patient follow-up and acts as a primary resource for patient caseload, facilitating services outside the healthcare facility.
- Develops and maintains productive relationships with colleagues and external partners, demonstrating accountability, enthusiasm, and cultural competence. Balances work and personal life, communicates safety hazards, and assists in care planning and practice improvements.
- Participates in quality improvement initiatives, supports patient satisfaction, and uses research to enhance clinical care. Assists with regulatory reviews and quality audits, advocating for patients within the practice.
- Establishes positive relationships with staff, providers, and patients. Works effectively in complex environments, handling multiple priorities, and demonstrating good clinical judgment. Communicates effectively, documents comprehensively, and incorporates professional standards to improve safety, quality, and satisfaction. Builds compassionate relationships with patients and families.
- Applies adult learning principles in teaching patients, families, students, and staff. Provides detailed education and develops total care plans. Supports staff development, may serve as a preceptor, and fosters an environment of continuous learning and inquiry.
