UPMC Community Medicine Incorporated is hiring a full-time OP Nurse Coordinator to support the Bone and Joint Center located at 300 Halket Street, Pittsburgh PA 15213. This role works Monday through Friday during daylight hours, with 2 days' work from home.

Responsibilities:
- Ensure accountability for professional development, participate in practice governance, and support quality improvement and change management
- Lead and oversee practice environment, assisting disciplines and directing clinical/administrative activities
- Formulate patient care goals and plans, involving patients as partners, and coordinate care with physicians
- Assist physicians with assessment, evaluation, and education in patient care
- Provide holistic patient care and follow-up to maintain health between visits
- Develop and maintain relationships internally and externally, demonstrating accountability and cultural sensitivity
- Support patient care planning, operations, and innovations with departmental management
- Participate in quality improvement efforts and clinical research to enhance patient satisfaction and safety
- Establish positive relationships with staff, physicians, and patients, managing multiple priorities effectively
- Apply critical thinking and clinical judgment, communicate effectively, and demonstrate physical stamina necessary for the role
- Identify and address clinical, social, safety, and psychological issues, ensuring comprehensive documentation and communication
- Educate patients, families, and staff, supporting professional development and serving as a preceptor if needed
