Job summary
House Rx is seeking a Specialty Pharmacist to join our Pittsburgh, PA HUB, serving patients onsite and collaborating with our centralized services team. You will optimize outcomes for specialty patients and enhance the overall pharmacy experience for patients and caregivers.
You will monitor therapies, manage prior authorizations and benefits navigation, and support access to medications while engaging with patients, providers, and care team members in person or by phone.
Qualifications
- PharmD and PA license to practice in Pennsylvania.
- Collaborate with care team to improve specialty outcomes.
- Navigate benefits verification, prior authorization and financial assistance.
- Engage with patients, providers, and care team in person or by phone.
- Able to build workflows in a startup environment.
Responsibilities
- Monitor and evaluate therapies for medication-related issues.
- Recommend therapy changes when needed.
- Monitor patient adherence and outcomes.
- Oversee care coordinator activities.
- Identify and resolve insurance-related issues for patients.
- Improve patient experience with world-class care and documentation.
- Act as liaison between patient and provider.
