Lakeland Care Solutions in Deltona, Florida is seeking a dedicated Physical Therapist for a full-time, onsite role in our rehabilitation facility. You will deliver evidence-based care, assess patients, and implement individualized treatment plans to restore function and improve quality of life.

Qualifications

• Bachelor’s degree in Physical Therapy from an APTA or CAPTE-approved program.
• Current state licensure in Physical Therapy.
• BLS/CPR Certification from the American Heart Association.
• Effective written and verbal communication skills.
• Proficiency in computer skills and EMR systems.
• Commitment to building strong patient relationships.

Responsibilities

• Evaluate patients and develop tailored treatment programs.
• Implement and oversee evidence-based treatment plans.
• Provide orthopedic, pediatric, neurologic, and sports rehab.
• Demonstrate strong customer service and patient rapport.
• Educate patients and caregivers about rehab processes.
• Collaborate with multidisciplinary team to improve outcomes.
• Meet or exceed utilization goals and contribute to program development.
