Job summary

Eastern Nephrology Associates, PLLC in Greenville, NC seeks a full-time clerical employee to coordinate patient care on-site. The role requires collaboration with providers, direct patient and caregiver interaction, and secure handling of health data in compliance with HIPAA. Work hours are Monday–Friday, 8:00 am–5:00 pm with occasional after-hours as needed.

Travel to other ENA sites may be required. A valid ADN/Health Sciences background and familiarity with HIPAA are essential.

Qualifications

• Associate degree in Nursing (ADN), Health Sciences, or related clinical field and 2 years experience, or an equivalent combination of education and direct patient care experience in nephrology or chronic disease management.
• Understand and practice the ethics of confidentiality, requirements of HIPAA.
• Valid driver's license and reliable transportation

Responsibilities

• Administer, record, and track Patient Activation Measures (PAM) tool to patients or their caregiver 25%
• Collect, track, and record patient information relating to dialysis starts 20%
• Track, collect and communicate data for patients at risk for end stage renal disease 25%
• Collect, track, and communicate information relating to kidney disease education 20%
• Other duties as assigned 10%
