Responsibilities

• Administer, record, and track Patient Activation Measures (PAM) for patients or caregivers (25%).
• Collect, track, and record dialysis starts data (20%).
• Track and communicate data for patients at risk of ESRD (25%).
• Collect and communicate information on kidney disease education (20%).
• Other duties as assigned (10%).
