UCHealth is seeking a Clinical Pharmacist for UCHealth Greeley Hospital in Greeley, CO. This onsite PRN role supports the GH Pharmacy department with direct patient care, medication safety, and interprofessional collaboration. Requires a PharmD/BS in Pharmacy and licensure as a Pharmacist.

Qualifications

• Pharmacist license required.
• Bachelor's Degree in Pharmacy from an accredited school.

Responsabilités

• Ensure medications are prescribed, accurately dispensed, and administered according to best practices.
• Provide drug information and counseling to patients and caregivers regarding medication use, dosage, side effects, and administration instructions.
• Stay current with updated formularies and treatment protocols through continuous professional development.
• Communicate with physicians and other healthcare professionals to facilitate collaborative care.
