Job Details

Looking to hire an experienced RN with AAS or BSN Degree and at least 2+ years’ experience in Hospice or Palliative care or med/surg, community health, and/or oncology nursing setting preferred

LOCATION - ONSITE at Skilled Nursing facility

RN wil see (4) hospice patients per day only

Once RN Patient visists are completed - RN will work from their own HOME OFFICE doing ptient charting and follw up calls to Physician, Social Worker and Spiritual Advisors.

RN will perform both physical and psychosocial assessments of patients in need of hospice or palliative care.

As the RNCM, you will electronically document visit occurrences.

Patient visits will be made in facility settings only - FREE onsite parking is availble at Skilled Nursing facility

You will develop and implement nursing Goals of care and make recommendations for care based on terminal patients' needs.

The work schedule is Monday thru Friday 830 am to 5 pm and includes working one weekend per month
