Job summary

Tallgrass Creek, part of Erickson Senior Living, is seeking a patient-centered Outpatient Physical Therapist to join our on-site campus. You will perform evaluations and provide personalized treatment across the continuum of care for older adults, with dedicated onboarding and strong documentation support.

All residents live on campus, no travel required, and you’ll work with an interdisciplinary team, including onsite physicians, to maintain mobility and quality of life.

Qualifications

• Must have a valid and current state Physical Therapy license.
• Experience with older adults in a clinical setting preferred.

Responsibilities

• Caseload focused on 1:1 direct patient care.
• Collaborate with interdisciplinary team to address resident needs.
• Maintain documentation and comply with state and Medicare requirements.
