Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You will document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist required.
• Experience with seniors in skilled nursing or post-acute care.
• Ability to work in an interdisciplinary team and document in EMR.

Responsibilities

• Evaluate residents to determine treatment needs.
• Develop individualized treatment plans to improve ADLs, mobility, cognition, and safety.
• Deliver evidence-based interventions and monitor progress.
• Educate families and caregivers on care plans and safety at home.
