Job summary

Hannibal Regional Healthcare System in Hannibal, MO, is seeking a Patient Monitoring and Chronic Care Management Coordinator (LPN or CMA/RMA) to work onsite, Mon-Fri, 8am-4:30pm.

Responsibilities include enrollment for RPM programs and Chronic Care Management, patient outreach and education, and coordination with the care team to ensure timely escalation of needs. Onsite position within the Care Management team.

Qualifications

• LPN or CMA/RMA required.
• Minimum of 2 years clinical experience.
• Care management experience preferred and/or willingness to become certified in care management.
• Strong verbal and written communication and facilitation skills required.
• Strong assessment skills, time management skills, critical thinking skills required.
• Must have ability to interact effectively with a variety of people and situations at all levels of the organization.

Responsibilities

• Enroll patients in digital remote patient monitoring (RPM) programs for chronic diseases (e.g., Heart Failure, COPD, Diabetes).
• Handle digital platform enrollment, alert monitoring, and patient education.
• Coordinate and escalate care; communicate with providers and multidisciplinary team.
• Enroll patients in Chronic Care Management and conduct outreach and education.
