• Provide one-to-one companionship and person-centered support to a 30-year-old high functioning individual with autism and mild intellectual disability.
• Assist the individual in community activities such as shopping, appointments, walks, events, and outings.
• Encourage independence, communication, decision-making, self-advocacy, and life skills.
• Help the individual work toward goals identified in the service plan.
• Provide reminders, routine support, or light assistance as required by the case.
• Maintain safety and follow individualized support instructions.
• Complete required documentation and communicate reliably with the Kindly team.
