OCES is seeking a dedicated One Care Plan Advocate to join the Senior Care Options (SCO) department. You will empower consumers to meet LTSS needs through assessment, person-centered care planning, advocacy, and ICT participation.
Responsibilities include developing ICP LTSS components, facilitating community LTSS, and promoting cost-effective, quality care plans within a supportive team. Hybrid work model offered with home visits.

We are expanding to 11 more towns covering the south shore area, including Quincy, Braintree, Weymouth, etc. OCES supports older adults and individuals with disabilities by providing vital information and coordination of services. By promoting healthy, safe living for our consumers, we hope to allow them to stay active and engaged in their lives and in their own communities for as long as possible. To that end, OCES is looking for a dedicated One Care Plan Advocate to work in our Senior Care Options (SCO) department to empower our consumers to meet their needs for Long Term Services and Supports (LTSS) through assessment, person-centered care planning, advocacy and participation in each consumer's Interdisciplinary Care Team (ICT); promote individualized care plans that are cost-effective and improve quality of life.
