Qualifications

• Master's Degree in Social Work is required.
• LSWAIC, LMHC, LMFT, LASW, LICSW, AAC may be required by department.
• Hospital/acute care experience preferred; social work in medical settings valued.

Responsibilities

• Administer assessment tools and gather data for assessment and services.
• Interview families to determine social work needs.
• Contact community resources to coordinate services.
• Provide ongoing assessment of social, emotional and cognitive needs, involving staff.
• Ensure accurate documentation for assessments, treatment plans and evaluations.
