Job summary

Advocate Health is seeking a full-time Clinical Social Worker to provide psychosocial support, discharge planning, and care coordination for patients and families. The role collaborates with the interdisciplinary team to address social, emotional, and environmental factors impacting care.

A CSWE-accredited MSW and proper licensure are required, with preferred ACM-SW certification and healthcare social work experience.

Qualifications

• Master's Degree in Social Work (MSW) from a CSWE-accredited program
• Clinical licensure or provisional licensure preferred
• Healthcare social work experience preferred

Responsibilities

• Assess patients' psychosocial and discharge planning needs.
• Develop and implement individualized treatment and care plans.
• Provide crisis intervention, counseling, and family support.
• Coordinate internal and external referrals and community resources.
• Advocate for patients and families to address barriers to care.
• Collaborate with the healthcare team on discharge planning and transitions of care.
• Coordinate care conferences as needed.
• Maintain knowledge of regulations, payer requirements, and reimbursement practices.
• Document assessments, interventions, and care coordination activities in the EMR.
