Qualifications

• Minimum two (2) years clinical RN experience in acute or ambulatory care.
• Bachelor's degree or equivalent required; RN license in California.
• Demonstrated ability to utilize utilization review, care coordination, and discharge planning.

Responsabilités

• Assess member needs and develop individualized care plans with physicians.
• Coordinate access to health services across providers and settings.
• Monitor care, coordinate transportation, and determine transfers when indicated.
• Act as liaison with outside agencies and document program changes.
• Educate patients and families to empower self-management of conditions.
