Job summary

Qualifications

Responsibilities

• Evaluates and identifies member needs with physicians to develop case management plans.
• Monitors effectiveness of care plans and makes modifications as needed.
• Coordinates interdisciplinary care, including transfers and authorizations for outside services.
• Acts as clinical liaison with external providers and agencies.
• Educates patients and families to empower self-management.
