Job summary

City of Hope is seeking a Pharmacist, PRN, to join its 24/7 inpatient pharmacy operations. This role requires flexibility to work all shifts as needed and contribute to safe medication use across the patient care continuum.

You will verify and assess medication orders, optimize therapies, document interventions, provide discharge education, supervise technicians, and collaborate with oncology teams and providers to ensure optimal patient outcomes.

Qualifications

  • Graduate of an ACPE-accredited College of Pharmacy
  • Active license with the State Board of Pharmacy
  • Doctor of Pharmacy or other advanced degree highly preferred
  • Minimum of 2 year’s institutional pharmacy experience or strong interest in institutional and oncology pharmacy

Responsibilities

  • Verify and assess medication orders for safety and appropriateness
  • Optimize medication therapies using clinical guidelines
  • Document pharmacist interventions accurately
  • Provide discharge education to patients and caregivers
  • Supervise pharmacy technicians
  • Collaborate in interdisciplinary rounds with medical teams
  • Support oncology treatment plans and oral chemotherapy counseling
  • Act as a liaison between providers and pharmacy to resolve drug-related issues
  • Assist with electronic health record (EHR) drug order workflow
  • Sterile compounding per USP 797/800 guidelines
