Job summary

City of Hope is seeking a Pharmacist to provide patient-centered care and integrated pharmacy services in both decentralized and centralized practice settings. You will review orders, screen for incompatibilities, and ensure cost-effective medication use in line with the COH formulary.

You will monitor chemotherapy regimens, educate providers and staff on drug information, and supervise sterile IV compounding with centralized and decentralized systems.

Responsibilities

• Reviewing and interpreting providers’ orders for all medications and ensuring cost‑effective use
• Screening for therapeutic incompatibilities and ensuring compliance with the COH Enterprise Formulary
• Monitoring drug therapy with emphasis on chemotherapy and providing drug information to providers and staff
• Overseeing technician staff related to sterile IV compounding, chemotherapy, TPN, and unit dose preparations
• Participating in quality improvement projects with emphasis on patient safety and engaging in multidisciplinary initiatives
