Qualifications

• Graduation from an accredited School of Pharmacy.
• State of Washington Pharmacist License.
• Two years of experience in pharmacy practice (may substitute residency months).
• PharmD degree preferred.

Responsibilities

• Collaborate with providers to individualize drug therapies for safe, effective care.
• Perform medication therapy reviews and safe transitions of care.
• Monitor and adjust regimens; order/interpret labs when needed.
• Provide refill authorization with adherence and safety assessment.
• Serve as drug information resource for the health care team and patients.
• Assist with access to specialty meds and insurer information.
• Educate patients on self-management and adherence.
