Froedtert Pleasant Prairie Hospital is seeking an Oncology Pharmacist to provide clinical pharmacy services in the Hematology/Medical Oncology Clinic, including education on chemotherapy and supportive care medications for patients and staff. The role includes order verification, patient counseling, and collaboration with the Cancer Center Infusion team.

The pharmacist will contribute to electronic health record expertise, ensure regulatory compliance, and support the 340B program.

Qualifications

  • Bachelor's Degree in Pharmacy required
  • Wisconsin Pharmacist License required
  • Entry level experience acceptable

Responsibilities

  • Act as liaison to Hematology/Oncology Clinic and Cancer Center Infusion.
  • Provide education on chemotherapy (IV and oral) and supportive medications.
  • Verify chemotherapy orders and support the Cancer Center team.
  • Counsel patients and families on regimens, adherence, and adverse effects.
  • Supervise interns/technicians; assist with dispensing and sterile preparation.
  • Participate in quality assurance, regulatory compliance, and drug utilization reviews.
  • Access and update patient information in the electronic health record.
  • Support Froedtert South 340B program utilization and compliance.

Job description

POSITION PURPOSE

Clinical pharmacy services to be provided within the Hematology/Medical Oncology Clinic. Those served include recipients and/or the family/caregivers of recipients of chemotherapy for cancer and its complications, as well as providers and nursing staff.

The oncology pharmacist provides education regarding the safe and effective use of chemotherapy, both intravenous and oral, and associated medications to the healthcare team, patients and families.

The oncology pharmacist facilitates the processing of chemotherapy and other orders for patients served in Cancer Center Infusion.

The oncology pharmacist identifies, solves and prevents medication-related problems in the hematology/oncology population.

The oncology pharmacist assists department leadership with chemotherapy-related regulatory compliance and financial considerations.

The oncology pharmacist, with appropriate competencies completed and maintained, may perform pharmacist duties related to chemotherapy dispensing or other staff pharmacist duties of the Pharmacy Department.

  • Residency or experience in health system oncology pharmacy is preferred.
PRINCIPLE ACCOUNTABILITIES AND ESSENTIAL DUTIES
  1. Act as the pharmacy liaison to the Hematology/Oncology Clinic and Cancer Center Infusion.
  2. Available to provider and nursing staff in both the Hematology/Oncology Clinic and Cancer Center Infusion for questions on medications including both oral and/or intravenous chemotherapy and supportive care medications.
  3. Provide electronic health record treatment plan expertise and support to the Cancer Center team.
  4. Provide individual patient/family counseling on chemotherapy regimens, adherence to chemotherapy and/or supportive care medication regimens and adverse effects of chemotherapy.
  5. Provides medication order verification and clinical services to the nursing staff, providers, and patients. These functions include floor/unit rotations; patient monitoring; admission history, value monitoring and active involvement on the code team.
  6. Work includes providing direct supervision of interns, technicians, and student work activities. These include dispensing; sterile products preparation; inventory control; repackaging and providing the professional check for these processes.
  7. Responsible for the control, dispensing, and maintenance of all medication used within the hospital. Is able to perform or assist in departmental tasks that are associated with these procedures.
  8. Participates in departmental or systemwide programs that work towards quality assurance and performance improvement. Participates in regulatory compliance. Collects data and evaluates medication events, adverse drug reactions, quality assurance and drug utilization review.
  9. Demonstrates the ability to access, enter and adjust patient information in the electronic health record.
  10. Works with Pharmacy leadership to support Department efforts to optimize Froedtert South's 340B program utilization, including but not limited to patient eligibility, program compliance, and contract pharmacy utilization.
