Job summary

Qualifications

• Bachelor’s Degree in Pharmacy is required.
• Current license in the state of practice.
• One year of specialty pharmacy experience is preferred.
• Knowledge of federal and state pharmacy regulations for long-term care.

Responsibilities

• Evaluate physician medication orders for appropriateness, dosage, interactions and route of administration.
• Maintain patient database with medical history, current medications and lab data.
• Ensure appropriate laboratory tests are ordered and discussed with the healthcare team.
• Collaborate with physicians, nurses and staff to provide clinical and technical support.
• Document all communications in the medical record.
• Implement policies for safe procurement, storage, distribution and disposal of drugs.
• Maintain compliance with regulatory documentation and reporting.
• Inform Pharmacy Manager of any medication-errors.
• Assist facility staff with safe drug handling and procedures.
• Perform other assigned tasks.
