Qualifications

Responsibilities

• Delivers cancer care across the continuum from diagnosis through treatment and survivorship and end-of-life care.
• Provides patient and family education and resources to support informed decisions and timely access to care.
• Clinically assess patients, provide education, and manage the complexity of cancer diagnoses.
• Communicates and collaborates with the multidisciplinary team.
• Orientates patients to hospital and outpatient settings and the organization.
• Assesses patient needs and reviews distress screenings for referrals.
• Supports physician and team communication and responds to new patient referrals.
