Job summary

Prisma Health is seeking an RN to coordinate oncology patient care across the continuum, supporting patients and families through the care journey at Greenville Memorial Hospital.

The role involves assessment, care planning, education, and collaboration with multidisciplinary teams to improve outcomes and access to services. A BSN is required with 3 years oncology experience; an RN license and oncology certification within 12 months are expected.

Qualifications

• Bachelor’s degree in Nursing is required.
• Three (3) years Oncology experience is required.
• RN license (compact/multistate) recognized by NCSBN or licensed in-state.
• Oncology certifications within 1 year of hire (OCN, ONN-CG, TCTCN, or CBCN)

Responsibilities

• Coordinate care for oncology patients across the continuum of care.
• Assess patient needs, barriers, and enrollment in cancer care management.
• Refer patients to support services after comprehensive assessment.
• Participate in multidisciplinary care planning and survivorship documentation.
• Educate patients and families to improve self-management and plan of care.
• Lead site-specific tumor boards and participate in cross-continuum process improvement.
