Job summary

MP RPO seeks an Oncology Nurse Navigator (RN) in New Hampshire to guide cancer patients from diagnosis through treatment and survivorship. You will educate patients, coordinate care among physicians and multidisciplinary teams, and act as a compassionate advocate for patients and families.

The role emphasizes documentation in the EMR, collaboration across services, and involvement in quality improvement and program development to improve patient outcomes.

Qualifications

• Current RN license in New Hampshire.
• Graduate of an accredited nursing program.
• Minimum of two years of clinical nursing experience.
• Current Basic Life Support (BLS) certification.
• Strong communication, organizational, and critical thinking skills.
• Ability to coordinate care across multidisciplinary teams.
• Compassionate, patient-centered approach.

Responsibilities

• Serve as the primary advocate for oncology patients and families throughout the care continuum.
• Educate patients on diagnosis, treatment options, resources, and care plans.
• Coordinate care among physicians, nurses, specialists, caregivers, and community resources.
• Identify and remove barriers to treatment by collaborating with providers.
• Follow up with patients after surgeries, hospitalizations, and treatments.
• Facilitate communication among clinicians and care teams.
• Coordinate hospice, home health, and supportive care services as appropriate.
• Manage documentation, care plans, histories, worklists, and results in the EMR.
• Support quality improvement, accreditation activities, and data collection.
