Job summary

The Christ Hospital Health Network is seeking a Clinical Nurse Navigator in hematology/oncology to guide patients through the cancer continuum, offering education, resources, and timely access to care. You will collaborate with schedulers, physicians, and other care team members, manage telephone triage, symptom management, patient education, and documentation using EMR/MyChart.

BSN strongly preferred; 5 years nursing experience; Oncology certification preferred and encouraged for eventual

Qualifications

• Graduate of an accredited school of nursing; BSN strongly preferred.
• Years of nursing experience: 5 years.
• Oncology Certification preferred; required to obtain within two years of hire if not already held.

Responsibilities

• Assumes nursing care responsibilities for a defined group of hematology/oncology patients, including assessment, education, coordination, and documentation.
• Point of contact for physicians, patients, and caregivers to aid in coordination of care.
• Supports transition from diagnosis and active treatment to survivorship and end‑of‑life care.
• Liaises with multidisciplinary teams to integrate patient information for seamless care.
• Triage patient calls, intervening as needed and alerting physicians when necessary.
• Follow up with patients to ensure adherence to treatment guidelines and monitor evolving conditions.
• Collaborate with Financial Advocates and Prior Authorization to facilitate access to treatment.
• Provide psychosocial support and facilitate referrals as appropriate.
• Develop care plans with physicians, referrals, and clinical trials; deliver patient education.
